RULES OF PROBATION AND COMMUNITY CONTROL
You are in custody of the Court by reason of your felony/misdemeanor conviction. Your rights as a citizen are restricted until your probation is terminated. As a probationer, you can be arrested without warrant by any Probation Officer or by any other police authority upon written order of your Probation Officer.
IT IS THE ORDER OF THE COURT THAT YOU SHALL COMPLY WITH THE FOLLOWING CONDITIONS OF PROBATION:
Report to your probation officer as directed. Complete the written report form accurately and completely.
Upon reporting to the Probation Department and when in Court you shall conduct yourself in an orderly manner.
Pay all restitution, probation supervision fees, court costs, drug testing and/or Home Arrest fees (if applicable) and other Court approved fees in regular monthly installments unless otherwise ordered by the court. Two consecutive lapses in monthly payments will result in notification of the court and/or a Probation Violation Hearing. All costs are to be paid in full 60 days prior to the expiration of your probation or a probation violation hearing will be requested.
Submit as directed to observed drug or alcohol testing. The only controlled substances you are permitted to have or to use are those properly prescribed by your doctor and you must provide proof of prescription upon request. You shall pay for the PSI Drug Screen and all subsequent screens as directed by the Adult Probation Department.
Submit as directed to evaluation and/or treatment for substance abuse, mental health, or other problems. You shall take medication as prescribed by your doctor and shall authorize release of information to the Probation Department.
You shall follow all rules and regulations of treatment facilities or programs of any type in which you are placed or ordered to attend while under the jurisdiction of the Medina County Adult Probation Department. If you are convicted of a DWI/DUI offense, you are prohibited from possessing or consuming alcoholic beverages while on probation.
Inform your probation officer immediately of any changes in your residence, employment, or source of income.
Do not associate with persons having known criminal felony or major misdemeanor records (including convicted co-defendants in your case).
Do not purchase, use, possess, have access to, or carry a firearm, deadly weapon or dangerous ordnance.
Do not leave your county of residence without permission from your probation officer. You must obtain written permission prior to leaving the State of Ohio.
Obey all local, state and federal laws. Report any new arrests or questioning by any law enforcement officer immediately to your probation officer.
You shall not enter into any agreement to act as an informer or a special agent of a law enforcement agency without the permission of the Court.
You may not work on any job caring for the elderly, children, or mentally disabled without the permission of the Judge or Probation Department.
Within 30 days of instruction in these rules, you shall obtain and carry on your person at all times, a valid photo identification issued by the State of Ohio, Federal Government, another State where you reside, or other identification approved by the Probation Department. You shall produce this identification upon request.
You shall waive your Fourth Amendment rights to search and seizure by your Probation Officer. Any search may include your person, vehicle, residence as well as any other property or area under your control at any given time and may be conducted either by your Probation Officer and/or any other law enforcement official.
Anyone convicted of a sex-related offense may not view, possess, or be associated with any type of pornographic materials including, but not limited to computer usage.
These conditions may be modified upon approval of the Court and/or the Probation Department. Offender will be apprised of any changes.
